This Privacy Policy and Terms of Use (collectively the “Agreement”) constitute a binding legal agreement between you and the City of Philadelphia’s Department of Behavioral Health and Intellectual disAbilities Services(“DBHIDS”), and are governed by the terms set forth at https://www.phila.gov/terms-of-use/ which are incorporated here by reference. This Agreement includes the access to and use of the website for HealthyMindsPhilly, an initiative of DBHIDS, located at https://healthymindsphilly.org and the screening programs, features and functionalities available therein (the “Site”). This Terms of Use describes the personal information that we may collect from you when you access or use the Site, and how we may use that information. By accessing and using the Site by any means (PC, mobile or other device), you consent to our collection and use of your personal information as specified in this Terms of Use.
Please read this Terms of Use carefully. By accessing and using the Site, you acknowledge and agree that you have read, understood and agree to be bound by this Terms of Use. If you do not agree to be bound by this Terms of Use, you should immediately cease all use of the Site.
1. What information is collected by HealthyMindsPhilly and how is it used?
HealthyMindsPhilly is committed to protecting the privacy of personal information to the extent possible, subject to applicable state and federal law. Other than as required by laws that guarantee public access to certain types of information, or in response to subpoenas or other legal instruments that authorize access, personal information is not actively shared. In particular, we do not distribute or sell personal information collected on our web servers.
We do not collect any protected health information, and we are committed to protecting your personal privacy. We do not require collection of any information about you that allows us or our sponsors (such as employers, health care companies, or universities) to identify you. Instead, we only collect basic demographic information about each user, such as their age, gender and marital status. This information does not allow us to identify you as an individual.
We collect this information to allow us to provide our sponsors with general, aggregated reports about the people using our site. These reports allow our sponsors to create services more targeted to the needs of their constituents. For example, we provide our sponsors with the percentage of men and women using our site.
When you complete a screening, we collect the responses to each question. We save these responses in order to provide you with the results of the test and to create the aggregated reports provided to our sponsors.
2. Does the Site use cookies and/or log files?
We do use cookies to improve your browsing experience and enable certain features or functionality.
We also measure statistics about visitors to the Site and their usage (e.g., where they came from, what pages they visit and what browser they are using). Knowing how our Site is being used means we can improve our users’ experience in the future. We do not use cookies for advertising purposes or pass user data to any other organization.
We do not collect general web traffic information about our users through log files. The company to which we outsource our web hosting function may collect the domains previously viewed by our visitors, but HealthyMindsPhilly does not obtain or share this information with any other entity.
3. What security protections are in place on the Site?
HealthyMindsPhilly has implemented generally accepted standards of technology and operational security to secure your personal information. To prevent unauthorized access and ensure the appropriate use of information, HealthyMindsPhilly has put into place physical, electronic, and managerial procedures that safeguard and secure our Site.
4. What policies apply to links out to third-party websites?
Occasionally, at our discretion, we may include, offer or link to third-party products or services on our Site. These third-party websites may have separate and independent privacy policies. We therefore have no responsibility or liability for the content, activities and data collection and use practices of these linked websites. We encourage you to carefully review the privacy policies of the websites you visit.
5. How can I contact HealthyMindsPhilly with questions or comments about privacy issues?

6. Children’s Privacy
The Site is designed to provide information and services for adults. The Site is not designed to attract minors, in particular children under the age of 13. HealthyMindsPhilly does not market to or knowingly collect information from anyone under the age of 13. Children should always receive permission from their parents before sending any information about themselves (such as their names, email addresses, and phone numbers) to anyone over the Internet.
7. Changes to our Terms of Use
We reserve the right, at any time, to revise, amend, modify or delete portions of this Terms of Use without prior notice.
