I usually am told, \u201cOh wow, that\u2019s a tough job\u201d or, \u201cOh, so you care for people at the end of life?\u201d Yes! Isn\u2019t that amazing?\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Palliative care is a type of medical care that helps with symptom management from chronic illnesses, and guides discussions around goals of care with patients. This type of decision-making allows the patients to decide on how they would like to proceed with their medical treatment. Sometimes patients decide that hospice care is the best option for them (Hospice care focuses on the <\/span><span style=\"font-weight: 400;\">comfort of a chronically ill, terminally ill or seriously ill patient&#8217;s pain and symptoms, while to their emotional and spiritual needs).\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">So, how hard is it to care for people at the end of their life? The answer, in a nutshell? It\u2019s very hard. Nursing is a calling; nurses are passionate about their profession. There have been several times in my career as a palliative care nurse practitioner that I have sat at the bedside of a dying patient and held their hand until they passed, or until family members were able to make it to the bedside. I\u2019ve had to tell patients that there is no more hope, that they are dying. I have cried with patients; I have shared in their anger about their new diagnoses. This is difficult.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">I was once told by a wonderful hospice nurse, \u201cYou only get one chance to die.\u201d These words constantly echo in my head. Sometimes the overwhelming pressure of making sure the patient is comfortable can really sit heavy on your soul. There is an emotional burden that nurses carry from simply empathizing with their patients. We almost internalize patients\u2019 emotions &#8211; which does, in my opinion, make us better caregivers, but it&#8217;s also a truly Herculean task.\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As nurses, we often must separate our work from our personal life. As personal as we may take a patient\u2019s death, we must also remember that it\u2019s okay to feel and talk about our emotions and how we are affected by the loss.\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Mental health for the patients during this critical time is essential. However, what about their nurse who is consumed by death and dying, sometimes multiple times a day, every day? For those of us who are in the helping profession we often feel that we are not doing enough for the people we help, we question our purpose, and feel guilty that at times, we do not have the answers.\u00a0 The emotions at end of life, the empathy, the pressure to make death a peaceful experience &#8211; this can all be very overwhelming.\u00a0<\/span><\/p>\n<p><img decoding=\"async\" class=\"lazyload aligncenter size-full wp-image-15026\" src=\"https:\/\/dbhids2021.wpengine.com\/wp-content\/uploads\/2019\/09\/CompassionFatigue.png\" data-orig-src=\"https:\/\/dbhids2021.wpengine.com\/wp-content\/uploads\/2019\/09\/CompassionFatigue.png\" alt=\"\" width=\"350\" height=\"350\" srcset=\"data:image\/svg+xml,%3Csvg%20xmlns%3D%27http%3A%2F%2Fwww.w3.org%2F2000%2Fsvg%27%20width%3D%27350%27%20height%3D%27350%27%20viewBox%3D%270%200%20350%20350%27%3E%3Crect%20width%3D%27350%27%20height%3D%27350%27%20fill-opacity%3D%220%22%2F%3E%3C%2Fsvg%3E\" data-srcset=\"https:\/\/healthymindsphilly.org\/dev\/wp-content\/uploads\/2019\/09\/CompassionFatigue-45x45.png 45w, https:\/\/healthymindsphilly.org\/dev\/wp-content\/uploads\/2019\/09\/CompassionFatigue-150x150.png 150w, https:\/\/healthymindsphilly.org\/dev\/wp-content\/uploads\/2019\/09\/CompassionFatigue-200x200.png 200w, https:\/\/healthymindsphilly.org\/dev\/wp-content\/uploads\/2019\/09\/CompassionFatigue-300x300.png 300w, https:\/\/healthymindsphilly.org\/dev\/wp-content\/uploads\/2019\/09\/CompassionFatigue.png 350w\" data-sizes=\"auto\" data-orig-sizes=\"(max-width: 350px) 100vw, 350px\" \/><\/p>\n<p><span style=\"font-weight: 400;\">When it all becomes too much, in the medical field we call this<\/span><a href=\"https:\/\/psycnet.apa.org\/record\/2012-03217-000\" target=\"_blank\" rel=\"noopener noreferrer\"> <b>compassion fatigue<\/b><\/a><span style=\"font-weight: 400;\">, (sometimes called secondary traumatic stress) which is described as the compounding emotional and physical exhaustion experienced by helping professionals and caregivers exposed to working with those suffering from the consequences of traumatic events. <\/span><\/p>\n<p><span style=\"font-weight: 400;\">It is usually experienced in nurses more than any other medical profession, but c<\/span><span style=\"font-weight: 400;\">ompassion fatigue and burnout can affect other people, including caregivers, public safety personnel, therapists, social workers, family caregivers and other mental health providers.\u00a0 <a href=\"http:\/\/transitionalsupport.com.au\/transitional-phase\/compassion-fatigue-trauma\/\" target=\"_blank\" rel=\"noopener noreferrer\">Although burnout is similar to compassion fatigue,<\/a>\u00a0there are differences. Compassion fatigue and burnout are both a form of stress that can occur anytime, but compassion fatigue usually stems from working with trauma victims whereas burnout happens over a period of time and is normally related to work stress and feeling overworked.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><b>Signs of compassion fatigue include:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Lack of sympathy or empathy for patients<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Exhaustion\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Anger<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Depression or anxiety<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Decreased passion for your career<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Sleep deprivation\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Dread coming to work<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">\u00a0<\/span><b>Signs of caregiver burnout include:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Sleep problems<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Feeling hopeless<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Lack of energy<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Changes in eating habits<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Becoming irritable, impatient, anxious<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Difficulty coping with day to day tasks<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">As a palliative care nurse, the prolonged exposure of listening to clients\u2019 traumatic stories makes one susceptible to compassion fatigue and is not always easily identifiable. Burnout is about being \u2018worn out\u2019 and can affect any profession. The impacts of burnout emerge gradually over time and are easily identified to direct links and stressors within the working and personal life. Things that inspire passion, drive and enthusiasm are stripped away as tedious; unpleasant thoughts take over<\/span><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If left unresolved, compassion fatigue (and burnout) can affect your work, your family, and eventually your mental wellbeing and physical health.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For those who are experiencing compassion fatigue, stress and\/or burnout, it is important to<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Understand that you are not alone<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Acknowledge how you are feeling<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Reach out to others and share your thoughts<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">You may need to break from the situation<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Seek professional help<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">Of course, there are other factors to compassion fatigue &#8211; like stress, heavy caseloads, or even lack of resources. Is this the cost of caring for patients? I sure hope not.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">After we recognize the signs of compassion fatigue, how do we get ourselves out of this rut? Sometimes it may be as simple as taking a mental health day, a week of vacation, any time to step away and rejuvenate.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The best things to do are try to separate your work life from your personal life. Try to leave work AT work. There may be times the weight is so heavy that there may need to be a refocus and the need to develop healthy habits for ourselves.<\/span><\/p>\n<p><b>Personal life focuses:<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Find a hobby (gardening, knitting, hiking, etc.)<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Journal! Get those emotions out on paper<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Exercise<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Have a spa day, or a day where you spoil yourself!<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Talk about your feelings with someone you feel close to<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Take the time to recognize the signs of emotional fatigue for yourself and develop healthy habits to deal with stress<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Don\u2019t over-commit! Have work\/life balance<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Debrief with management or with a bereavement counselor\u00a0\u00a0<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Develop a network of mentors, peers, and staff who understand your feelings and you feel comfortable discussing your emotions with<\/span><\/li>\n<li style=\"font-weight: 400;\"><span style=\"font-weight: 400;\">Seek help from a medical professional or psychologist\u00a0\u00a0<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">We are all human. Emotions affect us, and it may be overbearing. Nurses really love developing connections with their patients and even friendships; that\u2019s why we choose this profession.
